At Middle Path Wellness Collective, we believe the best client care happens through strong collaboration. Many of the individuals and families we support are referred by trusted professionals in our community, including physicians, school counselors, attorneys, coaches and fellow therapists like you.

Our clinicians provide thoughtful, evidence-based psychotherapy in a supportive and collaborative environment. We work with adolescents, adults, couples, and families navigating a wide range of concerns including anxiety, depression, relationship challenges, life transitions, trauma, grief, identity development, parenting stress, and mental performance for athletes.
Because we are a collective of clinicians with different specialties and approaches, we are often able to match clients with a therapist whose training and style fits their needs. Our team includes licensed clinicians as well as thoughtfully supervised pre-licensed therapists, allowing us to provide accessible, high-quality care while supporting the development of the next generation of mental health professionals.
When you submit a referral through the form below, our team will review the information and determine the best clinician match based on the client’s needs, availability, and insurance considerations. A member of our administrative team will then reach out to the client directly to guide them through next steps and scheduling.
If there are specific clinical considerations or coordination needs you would like us to be aware of, please include those details in the referral form. We welcome collaboration with other professionals involved in a client’s care whenever appropriate and with proper consent.

What Happens After You Submit a Referral
We strive to make the referral process as smooth and responsive as possible for both you and the client.
Step 1: Referral Submission
Complete the referral form below with as much information as you feel comfortable sharing.
Step 2: Internal Review
Our team reviews the referral and identifies the clinician whose expertise, availability, and insurance participation best match the client’s needs.
Step 3: Client Outreach
A member of our administrative team contacts the client directly to discuss scheduling, insurance, and next steps for beginning services.
Step 4: Scheduling the First Appointment
Once intake paperwork is completed, the client is scheduled for their first session with the matched clinician.
Step 5: Ongoing Care
If coordination with you or another professional is clinically appropriate and the client provides written consent, our team is happy to collaborate to support the client’s progress.
Our goal is to ensure every referral is handled with care, professionalism, and clear communication so that clients can begin receiving the support they need as quickly as possible.
